I understand and acknowledge that, while attending Princeton Photo Workshop Photo Camp for Teens, my child will spend each day learning and practicing photography, on several days walking to, from and spending time at, various local venues. While Princeton Photo Workshop will provide adult supervision during camp hours, I acknowledge that unpredictable hazards may occur. I am voluntarily consenting to my child’s participation in all activities and hold Princeton Photo Workshop and its affiliates harmless for accidental injuries to my child. I also will inform Princeton Photo Workshop of any aspects of my child’s physical and emotional situation or medical history that may affect their ability or willingness to participate in Camp activities, e.g., walking on pavement and on wooded trails, outdoor exposure to plants, exposure to foods, participation in group activities, communication with Campers and instructors, and any other factors that may impact my child's participation in Camp activities. I will instruct my child that he/she is responsible to exercise caution to avoid accidents, including wearing apparel and footwear appropriate to the day's activities, and to follow safety and other instructions given by staff. I understand that Princeton Photo Workshop staff members have the authority to immediately dismiss the child from a program if she/he is engaged in behavior that may be physically or emotionally unsafe for them or for others. I further give permission to Princeton Photo Workshop to use photographs and video that may include my child, as well as commentary about the camp willingly provided by my child, in their marketing. Princeton Photo Workshop will not publish the full names of any camper in marketing materials without prior consent. I AM SATISFIED THAT ALL REASONABLE CARE WILL BE TAKEN FOR THE SAFETY OF MY CHILD AND THAT ADEQUATE STAFFING AND SAFETY MEASURES HAVE BEEN ARRANGED. I HAVE READ AND FULLY UNDERSTAND THE INFORMATION ABOVE. BY ENTERING MY NAME IN THE BOX BELOW AND SUBMITTING THIS FORM TO PRINCETON PHOTO WORKSHOP, I ACKNOWLEDGE THAT I AM A LEGALLY RESPONSIBLE PARENT/GUARDIAN OF THE CHILD WHOSE NAME APPEARS AT THE TOP OF THIS FORM AND THAT I AGREE TO THE ABOVE TERMS.
